We suffered an outage on December 24th for at least four hours which prevented hospitals from entering data. The cause is resolved, and all operations are back to normal. We apologize, and are working to ensure this fault does not happen in the future.

Some users were not able to successfully activate their invites. We've resolved this bug and all invites are now able to create their accounts.
